したがって、本ガイドラインでは、専門家の合意により、がんによる神経障害性疼痛に対して、1種類の鎮痛補助薬を増量しても十分に効果がない場合は、専門家に相談したうえで、他の鎮痛補助薬への変更や併用を行うことを推奨する。

既存のガイドラインとの整合性NCCNのガイドラインでは、抗うつ薬 および/または 抗けいれん薬を第一選択とし、副作用が許容でき効果が得られるまで、常用投与量の範囲内(アミトリプチリンで50〜150mg、ガバペンチンで900〜3,600mg)で増量することを推奨している。脊髄圧迫症候群が疑われる場合には、迅速に専門家へ紹介することと、コルチコステロイドを投与することを推奨している。ESMOのガイドラインでは、オピオイドと抗うつ薬、抗けいれん薬の併用を推奨している。神経への圧迫による痛みの場合はコルチコステロイドの使用を推奨している。EAPCのガイドライン(2001a)では、オピオイドと抗うつ薬、抗けいれん薬を併用することを推奨している。脊髄圧迫症候群にはコルチコステロイド、放射線治療、化学療法を検討することを推奨している。ACCPのガイドラインでは、鎮痛補助薬(三環系抗うつ薬、抗けいれん薬)を使用し、無効な場合は専門家へ相談することを推奨している。新しく生じた腰背部痛に対しては、MRIで脊髄圧迫症候群の有無を精査し、放射線治療科、脊髄外科などの専門家へ迅速に相談することを推奨している。
